As a Consultant or Ambassador

I no longer wish to be a Consultant

If you no longer wish to be an Independent Consultant or Ôtentik Product Ambassador, please inform your assigned Consultant. A withdrawal form will be submitted for your signature. As of the date of receipt of the signed form, you agree not to speak on behalf of Ôtentik, nor to avail yourself of any other prerogative vested in Consultants.
